Intro to Forms and Form view

Forms gather all the info in one place so you can put it to work for your team! Use our intuitive form-building experience to quickly create Feedback, Project Intake, Order, Job Application, or IT Request Forms. Or start your Form from scratch and create whatever your team needs. 

After your Form starts receiving submissions, you can use ClickUp AI and Dashboards for real-time data analysis!

Create and access Forms

You can create a Form from the Views Bar, Forms Hub, and the Sidebar. You can open any Form from Forms Hub and the location where it was created. 

Add questions to a Form

After creating your Form, it's time to add questions!

Edit Form questions

From a Form question, you can edit things like the question's name and description, hide questions, and require questions to be answered. You can also access Custom Field settings from your Form.

Use conditional logic in a Form question

On the Business Plus and Enterprise Plans, you can use rules and conditions to add dynamic logic to your Form questions. Logic is used to guide Form submissions and capture more specific information. 

For example, you can create a rule to add a Due Date field in the submission task if Urgent Priority is selected in the Form dropdown.

Customize a Form's design

Customize the design elements of your Form by changing the layout, theme, and colors, or adding a cover image.

Form settings

In the Settings tab, you can apply a task template to or automatically assign Form submission tasks. There are also advanced setting options available for Business Plus Plans and above, like hiding ClickUp branding and adding a redirect URL.

Preview a Form before sharing

Preview your Form to ensure it looks right before sharing. Your Form is published by default and is ready to share. 

Access and download Form submissions

When people submit a Form, a submission task is created in the location selected when the Form was created. Learn how to access and download the Form submission tasks.

Share, embed, and export Forms 

Once you've created a Form, you can share a link to it with anyone, embed it on a webpage, or export your Form responses.

Use ClickUp AI for analysis

To use ClickUp AI for real-time analysis:

  1. Open the Space, Folder, or List where the Form
  2. From the Space, Folder, or List's location header in the upper-right corner click Ask AI.
  3. Ask AI a question about the Form submission tasks in that location.

Use Dashboards to visualize Form submissions

Create a high-level look at this info by converting Form submissions into Dashboards.

Submit Forms on mobile

Forms can now be submitted or shared using the ClickUp mobile app! You can tap a Form view from the Views modal to submit a Form response or share its URL from the mobile app.

Form views cannot be created or edited from inside the mobile app.
