You'll add questions to your Form using the Build tab:
Custom Fields and task fields
When people submit a Form, a submission task is created in the location selected when the Form was created. Custom Fields and task Fields are used to create questions. The field determines where in the submission task the responses display. For example:
- Task field type: The response displays in the corresponding task field. For example, Due Date.

New or existing Custom Field: The response displays in the corresponding Custom Field. For example, Date. New Custom Fields are also added to the Form's location so you can use them in any task in that location!
You can use private Custom Fields in Forms!
Forms can only use Custom Fields that exist in the Hierarchy where the Form is created. Forms created from the Sidebar have the following logic:

Information Block
At the end of the Question type modal, there's an Information Block layout option. Add an Information Block anywhere in the Form to include rich content like images or tables.
Add questions to a Form
When adding questions, you can create a new field or map to an existing one. For example:
- Forms support Number-type Custom Fields.
- I can select the existing Number-type Custom Field named Order number.
- I can select Number to create a new Number-type Custom Field.
- I can select the number-type task field Time Estimate.
To add a question or an Information Block to a Form:
- In build mode, at the bottom of the Form click Add question.
- You can also drag and drop Add question between existing questions to add a new one. Or drag and drop existing questions to reorder them.
- You can also drag and drop Add question between existing questions to add a new one. Or drag and drop existing questions to reorder them.
- Use the Search bar to search for a task field or Custom Field. Or select a Question type to see which fields are associated with it.
- Create new field: Create a new Custom Field of this type. Then add the response to this question to this new Custom Field.

Map to: Add the response to this question to an existing text field or Custom Field.
- In the Create field modal, you can edit the question name, add a description, and edit the settings.
Renaming a Custom Field on a Form only changes the name on the Form. The name of the Custom Field in other locations in your Workspace doesn't change.
