You can create a Form from the Views Bar, Forms Hub, and the Sidebar. You can open any Form from Forms Hub

Create and access Forms

Create and open Forms from the following locations:

Views Bar

Forms are also called Form view because they can be created in the Views Bar. 

A Form view created in the Views Bar can be accessed from its location's Views Bar or Forms Hub. 

To create a Form from the Views Bar:

  1. In the Views Bar, select Form.
  2. From the Create a Form modal, select a template or select Start from scratch to open an empty Form.
  3. Select the List where your Form submissions will be saved. 
  4. The Form opens and you can begin building it!

Forms Hub

A Form created in Forms Hub can be accessed from its Views Bar or Forms Hub.

A Space, Folder, or List in ClickUp 4.0

A Form created from a Space, Folder, or List can be accessed from Forms Hub, the Home Sidebar, or Spaces Sidebar. 

  1. In your Global Navigation, click Home or Spaces.
  2. Hover over a Space, Folder, or List and click the plus icon.
  3. Select Form.
  4. From the Create a Form modal, select a template or select Start from scratch to open an empty Form.
  5. The Form opens, and you can begin building it!

Create a Form from the create button

In ClickUp 4.0, you can create Forms from the create button in your Home Sidebar, Spaces Sidebar, and Forms Hub Sidebar.

To create a form:

  1. In your Global Navigation, select Home, Spaces, or Forms.
  2. In the upper-right corner of the sidebar, click the plus icon.
  3. Select Form.

Create a Form from the Sidebar in ClickUp 3.0

A Form created from a Space, Folder, or List in the Sidebar can be accessed from the Sidebar underneath its location or Forms Hub. 

To create a Form from the Sidebar: 

  1. In the Sidebar, hover over a Space, Folder, or List.
  2. Click the plus icon and select Form
  3. From the Create a Form modal, select a template or select Start from scratch to open an empty Form.
  4. The Form opens and you can begin building it!
